Visit PortalWe are pleased to share the SFCHC Board-approved “2023 Community Health Needs Assessment” (CHNA) conducted on behalf of the Center. As a federally qualified health center serving the most vulnerable, we are uniquely positioned to expand our services and overcome barriers to health equity – now and into the future. The data collected will help guide our future strategic planning activities, funding development and program growth. It also confirms the significant need for patients in our service area to receive high-quality, compassionate and culturally sensitive care that empowers them to make the best medical decisions. We are grateful for everyone's support as we continue SFCHC’s nearly century-long legacy of serving as a treasured healthcare home to the community we love.
Thank you for your support; it matters and helps make a difference every day.
