San Fernando Community Health Center, a Federally Qualified Health Center (FQHC) serving the Northeast San Fernando Valley is pleased to announce that we have received notice on July 21, 2020 that the Health Center was awarded an FCC grant of $112,244.00.  This funding is allocated to the expansion of telehealth services in response to the COVID-19 pandemic.  SFCHC will use these funds to provide vulnerable patients suffering from chronic diseases, such as diabetes, with connectivity to their health care providers while safely sequestering at home.
