We are a patient-centered medical home, where you will be provided with compassionate, high quality, and evidence-based care. The FQHC designation allows us to provide quality care at an affordable price to everyone in our community. SFCHC is located at the corner of Mott Street and Chatsworth Avenue, near public transportation and freeways with ample free parking available.
