San Fernando Community Health Center (SFCHC) is a state-of-the-art Federally Qualified Health Center located in the City of San Fernando and serving the entire San Fernando Valley. We are a patient-centered medical home, where you will be provided with compassionate, high quality, and evidence-based care. The FQHC designation allows us to provide quality care at an affordable price to everyone in our community. SFCHC is located at the corner of Mott Street and Chatsworth Avenue, near public transportation and freeways with ample free parking available. IMPORTANT: Our team is following all current Los Angeles County and state of California health & safety guidelines.

For all our valued patients, be aware that your earned benefits end in December. Then, everything resets in January with your annual insurance coverage enrollment. Need assistance with your coverage status? We can help.

Insurance & Payment

SFCHC accepts Medicare, Medi-Cal HMO, FamPact and other commercial plans. Services are available on a sliding fee scale which is determined by income and family size. If you have any questions about insurance or payment options, please call our office. Our friendly staff is happy to help you.

Medi-Cal Expansion

Enrolling in Medi-Cal means you will have access to health services for FREE or at low cost, inclusing preventative services - line annual check-ups, dental and vision care, and medication. Click to learn more.

SFCHC receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
