Program Spotlight: Community Public Health Teams

SFCHC has been selected to participate in the Los Angeles County Department of Public Health’s Community Public Health Teams (CPHT) pilot project. The goal of this program is to build a community-centered system of care that delivers coordinated health and social services to those most in need.

CPHTs consist of partnerships between community-based organizations and non-profit healthcare partners who work to identify local health priorities, reduce gaps in health outcomes and improve the community’s overall health and well-being. 

Throughout Los Angeles County, CPHTs will visit individual households to conduct needs assessments and link community members to critical resources and healthcare services. They also will collect and analyze health, mental health and social determinants of health data, as well as hold regular meetings in each targeted community. 

SFCHC will partner with Providence Health & Services, the lead agency on this project, and the Los Angeles County Department of Public Health for a period of up to five years. Through a shared leadership model, they will engage with local business, social service and healthcare organizations to serve approximately 9,000 people within a specific outreach area.

As a core CPHT member, SFCHC will help find new ways to share resources and referrals with vulnerable individuals and families, ensure activities are reflective of community needs and connect community members to health and social services.
