What is the Community Public Health Teams (CPHT) initiative?
The CPHT initiative is a new model of public health service delivery launched by the Los Angeles County Department of Public Health in collaboration with community partners. This pilot project aims to build long-term relationships with 80,000 households in Los Angeles County by providing outreach, engagement and services that support better health outcomes and eliminate health inequities.
Who are the partners involved in this initiative?
Los Angeles County Department of Public Health, in collaboration with
Rising Communities, is partnering with 10 community-based organizations and 10 healthcare partners in 10 high-priority, micro service areas throughout Los Angeles County. Each CPHT is a three-way partnership between a community-based organization, a healthcare partner and dedicated Public Health staff. Each team will coordinate to provide outreach, engagement, and services to at least 8,000 households.
What areas does the CPHT initiative cover?
The CPHT initiative covers select census tracts in 10 high-priority micro service areas throughout Los Angeles County, including neighborhoods in Lancaster, Pacoima, San Fernando, Mission Hills, El Monte, Boyle Heights, Florence-Firestone, Bell, Cudahy, Huntington Park, South Gate and Wilmington.
What role does SFCHC play in this program?
SFCHC is serving as the healthcare partner in the micro service area of Pacoima and San Fernando, with
Providence Health and Services as the community-based organization and project lead.
What strategies will the CPHTs implement?
The CPHTs will implement five core strategies:
- visit each household annually to gather information on household needs;
- ensure households are connected to requested health care and social services;
- analyze data on health, mental health and social determinants of health;
- convene community residents to identify health hazards and develop community action plans; and
- mobilize partners to advance health equity and improve community conditions.
How are CPHTs different from previous public health activities?
CPHTs emphasize a long-term partnership with community-based organizations and healthcare partners, expanding the public health workforce and building infrastructure for a community-based system of care.
Who funds the CPHT initiative?
The CPHTs and partners are partially funded by a Public Health Infrastructure Grant that was awarded to Los Angeles County Department of Public Health by the Centers for Disease Control and Prevention.
